Downloadable Forms for Existing and Potential Affiliate Counsellors


We currently have electronic e-mail version of Application Forms, Affiliate Counsellors' Manual, Clinical Policy, Therapy Closure Forms and other administrative documents Final or Partial- Long Case Account submission for payment by return.  If preferred they can be sent by post, or even filled in on line in complete security.  At the moment we have the following available for download and completing, the document will be encripted and returned to amian

Update September 09 - We are also looking for BACP/BPS/HPC! Counsellors/Psychotherapists Uk based or abroad, willing to offer clients a web based therapy, using video/audio contact via the web.  If you are interested please mention this on the application or call. All you will need is a computer and a broadband link. Our IT Dept will be happy to advise. This applies to UK based clients and those EXpats based abroad as well as locals employed by UK companies abroad.

Please note all therapy will be given under UK BACP/BPS Regulations

If you have a second language at a level sufficient for therapy please mention this as well.
